Case Manager
Now hiring for an Indianapolis Case Manager.
Responsibilities:
- Conduct intake assessments to determine client needs, health condition, and family support to identify possible resources
- Perform an eligibility screening to determine care criteria for public-funded programs
- Create and maintain care plans
- Maintain and complete required client documentation for program requirements
- Develop and maintain relationships with area organizations, community resources, and providers
Qualifications:
- Must be able to work 8am-4:30pm or 8:30- 5pm schedule
- At least one of the following backgrounds required
- Must have been employed as a Case Manager by Area Agency on Aging (AAA) since June of 2018, or a registered nurse, a license practice nurse, or
- An Associate’s degree in nursing with at least one year of experience serving old adults or disabled populations; or
- A Bachelor’s Degree in Social Work, Psychology, Counseling, Gerontology, Nursing or Health & Human Services; or
- A Bachelor’s Degree in any field with a minimum of two years full-time, direct service experience with older adults or person with disabilities (this experience includes assessments, care plan development, and monitoring); or
- A Master’s degree in Social Work, Psychology, Counseling, Gerontology, Nursing or Health & Human Services; or
- An Associates degree in any field with a minimum of four years full-time, direct service experience with older adults or person with disabilities (this experience includes assessments, care plan development, and monitoring)
This position is in office until hired in permanently and then would allow for some hybrid flexibility. All candidates must reside in Indianapolis or the surrounding suburbs.
